JOB DESCRIPTION
JOB TITLE: Case Coordinator
FLSA STATUS: Non-Exempt
RESPONSIBILITIES AND DUTIES
- Responsible for the overall coordination of HCS service delivery using an implementation plan based on the individual’s Person Directed Plan (PDP); and as outlined on the individual’s Individual Plan of Care (IPC); this includes meeting the deadlines as required.
- Integrating various aspects of services delivered under the HCS Program;
- Coordinate assessment of skills to support in establishing and outlining goals;
- Monitor the delivery of HCS Program services;
- Recording each individual’s progress or lack of progress on a quarterly basis;
- Record keeping; precise and accurate maintenance of consumer files;
- Arranging transportation requests and needs;
- Ensure that individual, LAR and service coordinators are informed of any changes in individual’s status;
- Ensure that individual, LAR and service coordinator are in agreement to any changes before implementing changes;
- Must be available to answer questions and provide support/guidance to individual and/or LAR;
- Coordinate the delivery of specialized therapy services as determined by individual needs;
- Coordinate the delivery of day program services as determined by individual needs;
- Coordinate adaptive aids and minor home modifications if determined necessary by the program and Service Planning Team for the health and safety of the individual;
- Work with the nursing staff to ensure adequate monitoring of individual’s health conditions as needed/applicable;
- Support individual with residential setting of choice;
- Promote positive involvement of LAR/family member(s);
- Adhere to Case Coordinator Protocol at all times;
- Adhere to On-Call rotating Case Manager schedule (if applicable).
This position is the single identified person accountable to the individual and LAR for coordinating the individual’s overall program. This position involves the exercise of independent judgment and discretion regarding programmatic matters, ensuring that all of the HCS principles are adhered to. Case Manager must be available to render services at any given time when necessary, during business hours. Reliable transportation may be required in order to conduct home visits. A valid Texas Driver License is required while driving company vehicles.
